The Free Prostate Specific Antigen (Free PSA) Test is a blood test that measures the amount of PSA circulating freely (unbound) in the bloodstream. PSA is a protein produced by the prostate gland, and it exists in two forms—bound and free.

This test is often used along with the total PSA test to calculate the free-to-total PSA ratio, which helps differentiate between benign prostate conditions (such as benign prostatic hyperplasia) and prostate cancer. Lower ratios of free PSA may be associated with a higher risk of prostate cancer.
