IgM antibodies can develop as part of the body’s immune response to HSV. However, HSV IgM testing has significant limitations. A positive result does not reliably indicate that an infection is recent, because IgM may occur during recurrent infections and the test may not reliably distinguish HSV-1 from HSV-2.

Therefore, the result should be interpreted together with symptoms, medical history, exposure history, and other laboratory findings. If active blisters or sores are present, HSV PCR/NAAT testing from the lesion is generally more useful for confirming an active infection. Type-specific HSV-1 and HSV-2 IgG testing may also be considered for assessing established infection.
